Many organisations lack the expertise or confidence to thoroughly address the behavioural and non-technical causes of incidents. We can help you fix real problems through our Human Factors investigation expertise and training courses. Rather than concluding an investigation with “human error” or trying to identify a single human “root cause”, our team will help you identify the underlying causes.

We can support you at all stages in the investigation process, with varying levels of involvement. We have hands-on experience of doing this, and our services are practical and solution-oriented. Prior to an investigation we can train investigators in human factors evidence gathering, interviewing techniques and human factors analysis. During an investigation we can provide a human factors specialist to support or coach the investigation team with evidence gathering, analysis and developing recommendations. We can conduct independent reviews of completed investigations to identify additional human factors learning, or to examine a series of repeat incidents to understand common behavioural causes.
